Smile And Save Dental Annual Membership Agreement Terms & Conditions

West Morris Dental, PC is a dental practice which provides dental services at 23 US-46 East in Hackettstown, NJ 07840. In exchange for certain fees, the dental practice agrees to provide you with dental services permitted to perform under the laws of New Jersey. All individuals purchasing memberships with West Morris Dental, PC (WMD) should understand these terms and conditions.

Disclosures: This is a membership plan and not dental insurance. Smile and Save Membership plan provides discounts for certain dental services not goods or products purchased at WMD.

Term: This agreement will last one (1) year, starting on the date the agreement is signed and paid. It is non- refundable. Membership includes coverage for the applicant only and can only be honored at West Morris Dental, PC in Hackettstown.

Renewal: The agreement will automatically renew each year on the anniversary date of the agreement. WMD is not required to give notice that your membership is expiring. You may cancel your membership in advance in writing with no less than 15 days’ advance notice.

Policy: WMD has the right to cancel memberships before duration if they deem there has been an unauthorized user. You cannot utilize WMD’s Smile and Save Membership if you have any dental insurance plan or dental coverage. It cannot be used in addition or in conjunction to any discount plan or third party.

Fees: When requested, WMD will provide the costs of services for any procedures beforehand. Each member is entitled to receive discounts on specified services. Members must pay the provider at the time of service to receive the discounted fee. WMD Fee Schedule is subject to change without notice. Fees quoted will be honored for 90 days.

Limitations and Exclusions: Appointment no shows or untimely cancellations (less than 24 hours advance notices) may void the membership and no refund will be made. Services which in the opinion of the attending dentist are neither necessary nor recommended for the patient’s health, need to be referred to a specialist (a non-participating dentist), lost or theft of dentures or bridgework, and/or services covered under Worker’s Compensation, Employers Liability or No-Fault Auto Insurance laws.

Membership fees and plan discounts are subject to change.

WMD reserves the right to cancel or discontinue this plan for any reason at the end of the membership term. Membership services are used or lost; they cannot be carried over.
